Please disable the plugin once and reactivate it. This will add the missing database columns.

Installation
Upload the plugin to the “/wp-content/plugins/” directory.
Activate the plugin through the “Plugins” menu in WordPress.
Edit the contact formular you want to add the double opt in System.
